What do you think of rental car companies?

Everyone uses them but nobody is posting anything here. I run a classic car hire company so I'm really interested to see what you think makes a good rental car experience, and what makes a bad one? When your car rental experience has been bad, what has it been like? Ripped off by extra charges? Poor quality car? When it's been good, why has it been good? Has it actually EVER been good or just okay?
